Relocation Services

Relocation Service Coordination (RSC) are activities coordinated and designed to help a person who resides in an eligible institution to gain access to needed medical, social, educational, financial, housing and other services and supports that are necessary to move from an eligible institution to the community. We work with the entire state of Minnesota.

    Transitional Services

     

    To assist eligible individuals in the acquiring the items and help with the expenses necessary and reasonable for a person to transition from a licensed setting to his or her own home

    Requirements

    1. Two weeks of advance notice (RS Policy)
    2. A person’s own home is a setting that he or she owns, rents or leases that is not operated, owned or leased by a provider of services or supports. (DHS Policy)

    Housing Access Coordination

     A waiver service that helps people plan for, find and move to homes of their own that are not owned, leased or controlled by disability services providers. It is a pay-for-performance, person-centered service that pays for staff assistance based on the results achieved. A provider is reimbursed for actual time spent helping a person get housing.
