Case Management Services

Case management Services assist individuals with disabilities gain access to and navigation of social, health, educational, vocational, and other community and natural resources. Services are based on a person’s values, strengths, goals, and needs. Waiver Case Managers are responsible for providing people with information to make informed choices.

Waiver case managers provide individuals and their families with access to an assessment, person-centered planning, referral, linkage, support plan monitoring, coordination and advocacy related to waiver services, resources and informal supports that are not necessarily funded through the waiver.

Redeemer Services, Inc.’s case management team consists of experienced and diverse professionals who work collaboratively sharing their knowledge and insight.
